NAPEBT annually inquires partners about changing medical insurance structures. City of Flagstaff Human Resources presented the option to EAC. EAC voted to ask employees directly

Definitions Tier: a row, layer, or group; a category or type of family Tiered Medical Insurance: insurance coverage based on family/dependent types Subsidy: City contribution for dependent insurance Deferred Compensation: money distributed to employees who do not insure dependents Proposed Dependent Subsidy: money provided to compensate for an employee covering dependent(s) based on family/dependent type Insurance Premium: cost for insurance coverage Employer Cost: amount paid by employer Employee Cost: amount paid by employee

The anonymous numbered surveys will be sent to employees through EAC Representatives Hard copies of the completed surveys should be returned to your EAC representative directly Questions on the survey are the following: Do you want to modify the existing medical insurance structure? If modified, which tier structure would you prefer 3 tiers or 4 tiers? Do you prefer the current subsidy or proposed subsidy?

What are tiers? Tiers are categories of employee family types. Existing 2 Tier Medical Insurance Structure Employee only Employee and Family Possible 3 Tier Medical Insurance Structure Employee only Employee plus child or spouse/ domestic partner Employee and family Possible 4 Tier Medical Insurance Structure Employee only Employee plus spouse/ domestic partner Employee plus child or children Employee plus family

Dependent Subsidy $254.00 per month - amount City of Flagstaff currently contributes to each employee per month for medical insurance.

Proposed Dependent Subsidy $50 or $314 per month – amount City of Flagstaff may contribute to each employee per month in a 3 Tier Structure for plus one or plus family respectively $25, $75, or $320 per month - amount City of Flagstaff may contribute to each employee per month in a 4 Tier Structure for plus child/children, plus spouse/domestic partner, plus family respectively
