www.saferhealthcarenow.ca Falls Facilitated Learning Series Lessons Learned on Sustaining Falls Improvement Work during Action Period Need to make our Falls Team a priority (too easy to do other work, causing us to fall behind) Maintain the time and commitment we agreed towards the Fall Team, maintain weekly meetings Use of the Morse Falls Scale is being used on all admissions with 100%, key tool for our Home Care criteria
