Operated by Los Alamos National Security, LLC for NNSA U N C L A S S I F I E D Slide 4 CAO-AM Assessment Results Total Chemical Sample = 270 Total Errors = 133 Percent Error = 49% Percent Accurate = 51% Breakdown of Errors Missing Barcodes = 2 Chemicals present in the building but not listed in ChemLog = 31 Chemicals listed in ChemLog but not found during the facility tour = 98 Possible Causes for the Error 72 of the 98 not-found chemicals from our original list were listed in ChemLog under Obie Gillespie as owner, but according to Doris Ford, the chemicals were never in the room. In 7/21/04 Doris Ford submitted a message requesting that they be removed from the ChemLog list, explaining that the chemicals were never in the room. In 10/03/07 Obie Gillespie resubmitted the request. They are still on ChemLog.

Provide Barcode Readers that are allowed at TA-55. Require Chemical Owner be present during an Assessment.

Institutional expectations of ISD101-14, Chemical Management and the Technical Safety Requirements of Section of TA-55-PED , TA-55 Technical Safety Requirements (TSRs) are implemented through this program.
