Tamara Maciel Bannan, MPH Director, QI On-TAP June 11, 2015 COMMUNITY HEALTH IMPROVEMENT PROCESS: ENGAGING THE COMMUNITY

TODAY’S CALL Welcome and introductions Presentation on community engagement Community engagement tools and resources

PHASE 1: ORGANIZE FOR SUCCESS/PARTNERSHIP DEVELOPMENT MAPP is initiated by lead community partners coming together in an organized manner to prepare for planning Requires high level commitment from partners, stakeholders, and residents Health departments must continue to cultivate relationships with these entities and individuals to keep them engaged Successful engagement will lead to a successful implementation of a community health improvement plan

3 Common Challenges 1.Resource limitations Staff Time Financial 2.Loss of momentum 3.System partners leave implementation to the local health department

Ensuring Success Leadership support Focus on system throughout process Leveraging partner resources (e.g. business community) Neutral facilitators Clear goals, objectives, action plans, and accountability Money follows good planning

ACCREDITATION READINESS The MAPP process is used in developing Community Health Assessments (CHAs) & Community Health Improvement Plans (CHIPs) CHAs & CHIPs are two of three documents required to apply for national Public Health Accreditation
