Founded in 1974, HBHC is a Chicago-based health care and research organization, primarily serving the lesbian, gay, bisexual, transgender, and questioning/queer (LGBTQ) community and its allies, with over 27,000 patients/clients seen in Awarded Federally Qualified Health Center (FQHC) status in August 2015 Services include:  Primary Medical Care  HIV/STI Prevention  PrEP/PEP  Behavioral Health  Research  Walk-In Clinic  Youth Services  Case Management  Elder Services  Women’s Health  Brown Elephant Resale Shop

Couple meets with Patient Service Representative (PSR) Registers for either Walk-In Counselor or Walk-In Nurse visit Each receives Relationship Health Assessment Forms Each person completes forms while waiting to begin screening process Forms include screening component for intimate partner violence (IPV), coercion, and individual sexual health history Registration Relationship Health Assessment Forms Walk-In Counselor or Walk-In Nurse Couple is called back together into an exam room by either a trained test counselor or Registered Nurse Screening and specimen collection is completed, HIV rapid test is administered in lab Testing Together HIV Testing and Counseling     After reviewing the Relationship Health Assessment Form for IPV/coercion screening, test counselor makes a decision on whether to proceed or meet individually Testing Together HIV testing and counseling session is completed, referrals/linkage provided if necessary

Partner & Relationship Characteristics o Screening for intimate partner violence (IPV) and coercion Individual Sexual Health Assessment HIV Testing Together Consent Form Each person has EHRS chart established during registration process “TT HIV Clinic Testing” form is created within chart o Mirrors Sexual Health Assessment information collected on forms o Test counselor will manually input responses after visit is complete o Forms scanned and imported into persons chart Standardize data and easily document outcomes
