SC Success Story Team Approach Merger of two departments: Lifeline & Home Health Good Communication between Lifeline staff, field clinicians and clinicians reading daily vitals Focus is on Congestive Heart Failure, Chronic Obstructive Pulmonary Disease, and Hypertension Weekly updates to physicians

SC Success Story The Positive Approach to Installation Help patients become comfortable and confident taking vitals Teach the patients what the numbers mean Help build the patients confidence of maintaining condition Reinforce that the patient should continue taking vitals once the Telemonitor session ends

SC Success Story Perfecting the Installation The dirty work Looking for roaches at install – living in Charleston SC it’s just a fact of life. They will ruin the system by getting inside. Cleaning equipment – sterilizing equipment is time consuming but it needs to be done on a regular bases so you don’t end up with a waiting list. Managing equipment – In the event a patient expires or moves it’s important to gather the equipment quickly before it’s lost. It’s amazing how quickly that can happen.

SC Success Story Choosing Telemonitor Equipment Choose equipment with that offers a scale with a handle bar to meet the needs of patents and provides safety Software that offers reports showing # of ED visits, # of Readmissions, and # of compliance to show success or failure of program

SC Success Story Choosing Telemonitor Equipment Equipment allows patient to take vitals at different times Weight immediate after waking and emptying bladder, BP an hour after taking medications, allows patient to take 02 readings before and after breathing treatments Software that shows patients risk stratification for re- hospitalizations based on survey answers

SC Success Story Success by Numbers Reporting Period 01/2014 – 10/ of patients served 29 of patients had emergency room visits 48 of patients had 30 day readmits to the hospital

SC Success Story THANK YOU!
