B E CONSULTING SOLUTIONS,LLC. | Zylast Skin Irritation While CHG is one of the more powerful antimicrobials, perhaps the biggest drawback in using CHG-based products is the skin irritation associated with them. Contact dermatitis is experienced by more than 50% of healthcare personnel, and is a leading cause of failures in hand hygiene compliance. In testing, only Triclosan was found to be more drying to the skin than CHG with repeated use. The active ingredient in Zylast, BZT, was actually found to increase moisture levels in the skin with use, protecting the skin from damage and encouraging use (Paulson et al). Not surprisingly, the drying effects of CHG result in greater skin irritation. When used 100 times daily for 5 consecutive days – mimicking use in healthcare facilities – a 4% CHG solution was found to be the most damaging to the skin. Even 0.75% CHG, less than in Avagard and most CHG soaps, resulted in significant irritation. Of the ingredients tested, only BZT did not result in any irritation for the user after 100 daily uses for 5 consecutive days.

The testing was performed against a 4% CHG scrub, the same active ingredient as HibiClens and other standard soaps used in healthcare settings. Zylast Foaming Soap significantly outperformed all products, including the CHG scrub, against both bacteria.

B E CONSULTING SOLUTIONS,LLC. | Zylast Deactivation by Other Products Concerns have been raised about the deactivation by CHG by other hand hygiene products, moisturizers, lotions, and other products. Among the major concerns is an item Carbomer which is an anionic thickener used in many alcohol-based hand sanitizers, including Purell. Purell Advanced™ contains citric acid, cellulose, potassium sorbate, sodium benzoate and lactose – all ingredients that deactivate CHG products. Facilities that use alcohol-based sanitizers and CHG soaps may be accidentally reducing the effectiveness of their own hand hygiene products. Researchers noted that the two most common thickeners in alcohol-based products, carbomer and acrylates, both inhibit CHG. This is not a small effect, either.

In the presence of an alcohol-based sanitizer containing carbomer, the efficacy plummeted to 0.54 log reduction – more than 1,000 times less effective in killing bacteria!
