Problem Classification Scheme The Problem Classification Scheme provides a structure, terms, and system of cues and clues for a standardized assessment of individuals, families, and communities. Area of Focus

Intervention Scheme Your Actions The Intervention Scheme is designed to describe and communicate multidisciplinary practice, practice that is intended to prevent illness, improve or restore health, decrease deterioration, and/or provide comfort before death. Your Actions

Problem Rating Scale for Outcomes The Problem Rating Scale for Outcomes is a method to evaluate client progress throughout the period of service. Outcomes

Creating a Pathway Three steps to creating a new pathway: What is the 1st, 2nd and 3rd Step? Identify the client population/group (title of the pathway) Choose only problems that are most commonly experienced by that population. Add typical interventions related to those problems. Define your guides (assessment / flow sheet data to be collected)

Why are Pathways Effective/Useful? Represent generic client populations that you serve. Articulate best practice based on state/program requirements and/or standards of care. Effective tools to train or guide staff on process or procedure. Document data consistently. Number One Reason to use Pathways? Outcomes!

Flow Sheets Documentation tool used outside of narrative documentation to quickly document care for a client population. Definition: a patient care record that documents interventions through the use of check marks and brief notations. Question: How do we recreate the simplicity of a flow sheet in NN using the Omaha System?

Omaha System Pathways Great marriage of the convenience of a flow sheet but utilizing a standardized language. Why is use of a standardized language important? Outcomes! Ability to report data in a meaningful way in comparison to other users of a standardized language. Ability to share information with other providers of care through HIE’s. Through Big Data..our data will dictate future practice!

Pathways as Flow Sheets Suggestions when Developing Pathways: Try and group like interventions under one main Problem(Area of Focus) If you or your agency wants to measure outcomes of some aspect of care remember you will need to pull it out as a separate problem For all postpartum clients you assess their nutrition. Your agency wants you to track outcomes for nutrition. You would identify the problem of Postpartum and Nutrition. Build answers or results into your guide +nits / -nits Low lead level (0-3) / Moderate lead level (3-4) / High lead level (5+) Use terms such as normal vs abnormal; wnl Include hints/suggestions within the guide as what narrative is expected Monitor and identify changes in the following: Income, Education, Employment, Transportation, Living/resident status, Other (specify)
