Challenges of the present – health promotion / education

Primary health care is … accessible to individuals and families … It is the first level of contact … bringing health care as close as possible to where people live and work... Declaration of Alma-Ata, 1978

Challenges of the present One GP model – overload - lack of time for prevention – isolated working circumstances Different accessibility - chronic care and rehabilitation service based on outpatient services Enormous administration - lack of IT support

Definition of „ General Practitioners’ (GPs’) cluster: a network of collaborative primary care practices that, in addition to offering the traditional curative, acute and emergency services, provides preventive services and health promotion interventions. The goal of the GPs’ cluster: to improve the health status of the serviced population not only by providing care for the sick but also by providing services and activities aimed at preventing diseases and improving the health status of the entire target population.
