Nutrition

Triggers for Nutritional Services Referral  Growth and Weight Management  Health Conditions  Needs for Specialized Supports  Economic Problems  Behavior Problems

Role of the Services Coordinators Regarding Nutrition Services  Determine Waiver eligibility  Assess/identify nutritional needs  Discuss needs and explain benefits to client/family  Authorize services  Develop Nutrition Plan with client  Contact Resource Developer for names of nutritionists in community  Determine who will contact Nutritionist

 First visit: Assessment & Development of Nutrition Plan  Up to 5 additional visits within 3 month period as indicated in Plan  At end of authorization period can authorize 3 additional months if needed Authorization of Services by Services Coordinator

 Assess Client’s Nutritional Status  Develop Nutrition Plan with Client  Counsels, Intervenes, Follow Up  Provides Documentation to Client and Services Coordinator Role of Licensed Medical Nutrition Therapist (LMNT) or Certified Diabetes Educator (CDE)

Who Is Involved in the Development of Nutrition Plan ?  Nutritionist  Client  Services Coordinator  Other team members (family members or friends)  Client’s Physician

Nutrition Plan Includes:  Physician Identified Areas of Need  Client’s Strengths, Needs and Desired Outcomes  Specific Activities  Who Does What  Specific Nutrition Outcomes  Number of Visits per Month  Areas of Teaching

Who Receives Copy of Nutrition Service Plan  Client’s physician if physician-identified needs  Client  Services Coordinator  At beginning when the Nutrition Plan is initiated  On-going communication  At end of three month authorization period

 Services Coordinator receives progress outcomes summary after initial 3 months  Services Coordinator may authorize an additional 5 visits over next 3 months Monitoring the Nutrition Plan

Services must be terminated when:  Outcomes of Nutrition Plan have been met  Client is clearly not benefiting from Nutrition Services  Client requests termination of the service Termination of Nutrition Services

Current Nutrition Service Rates are set by Central Office  Be sure to check with supervisor for current rate amount  Rates may change annually Nutrition Services Rates

The Nutrition Service Provider must maintain a file that contains:  Nutrition Plan and all recommended changes  Progress reports  Name of the client’s physician  All pertinent medical information Record Keeping

 Maintains an active list of Nutrition Service Providers  Provides a current list of the Nutrition Service Providers to client  Recruits new Nutrition Service Providers Role of Resource Development
