SCHOLARS FUND PROGRAM AND REQUIREMENTS 2

Scholars Fund Overview Provides scholarship awards to eligible student employees to assist with undergraduate expenses -expenses include: tuition, books, lab fees and room and board To remain eligible for the fund, Scholar Candidates must complete all steps (and deadlines) as outlined in the Program Description Awards are given of up to $750 per semester, for up to eight semesters (maximum $6000) for eligible scholars attending a college, university or trade school 3

Requirements Must have a minimum cumulative grade point requirement of 2.0 on a 4.0 scale Graduate from high school by June 30, 2016 Complete an essay, application and resident nomination according to deadlines Should plan on enrolling full-time (at least 12 credits) in an accredited undergraduate college, university or trade school 4

Requirements Work a minimum of 1,000 hours for ACH before qualifying as a Scholar Maintain continuous, year-round employment through June 7, 2016 Be “in good standing” Complete each step as outlined in the Program Description, by the due date, to remain eligible for this fund 5

Program Requirements Attend mandatory informational meeting (go over program policies and have photo and video taped message taken) DUE DATE: 11/30/16 Submit a nomination form from resident. DUE DATE: 03/01/16 Complete Scholars Fund Application and Essay. DUE DATE: 3/01/16 6

Program Requirements Submit a Department Recommendation Form, completed by your supervisor and department director. DUE DATE: 3/01/15 Becki Nixon, Philanthropy Coordinator, will verify hours. The Scholars Candidate will be notified of any discrepancies. DUE DATE: On May 31 st (if less than 1,000 hours). 7

Program Requirements Participate in a mandatory meet and greet with residents during April of DUE DATE: April 2016 Meet and Greets will be: Tuesday, April 5 th 4-6pm in all restaurant lounges Wednesday, April 13 th 4-6pm in all restaurant lounges Monday, April 18 th 4-6pm in all restaurant lounges Wednesday, April 27 th 4-6pm in all restaurant lounges (Students will need to dress up and resident volunteers will be there to assist) 8

Program Requirements Scholars Candidate MUST wear Scholars Candidate pins or Scholars T-shirts whenever they are on campus and on duty during specific dates identified below. DUE DATES: April 1 through May Be on ACH payroll at the time of the Awards ceremony on June 7, NO EXCEPTIONS. 9

Program Requirements ALL SCHOLARS MUST ATTEND the Scholars Program Awards Ceremony and Reception. DUE DATE: Tuesday, June 7 from 7-9pm Submit college acceptance letter to the Philanthropy Department. DUE DATE: 7/31/16 Submit a copy of your final high school transcript upon completion of your senior year. DUE DATE: As soon as possible, but no later than 8/15/16 10

Program Requirements Write a Thank you note to residents. This note must be typed in black ink, addressed to the Ann’s Choice Residents, and signed by the Scholar Candidate. Grammar, punctuation and spelling should be correct as this will be available to all ACH residents. Submit thank you note to the Philanthropy Department (located in Liberty Commons). DUE DATE: 6/30/16 11

How are the Scholarship Awards Distributed? 13 The award is paid directly to the accredited undergraduate college, university or trade school where the student enrolls after high school graduation

14 Processing Your Scholarship Each semester mail in your scholarship form and proof of full-time status (official copy of tuition bill, official copy of your schedule or an official letter from the school stating you are a full-time student for that semester) to: Ann’s Choice Philanthropy Department Ann’s Choice Way Warminster, PA 18974

15 Processing Your Scholarship Turn in your paperwork by the deadlines: Fall Semester – September 15 Spring Semester – February 15 Please Note: If your paperwork is received after these dates you will not be eligible to receive your scholarship money for that semester

16 Processing Your Scholarship Avoid turning in your scholarship information to any other person or department other than the Philanthropy Department and follow-up with us to make sure we received your information. Please mail to us. Ann’s Choice has 60 days to get your scholarship to the school (but it usually takes days) If you need us to write a letter to the school on your behalf stating that the scholarship money will be sent to the school please call us and we will be happy to help you

CONTINUING TO RECEIVE THE SCHOLARSHIP 18

Can Scholars be discontinued from the Program? YES, if any of the following occur: Fails to meet grade requirements Fails to meet all steps as outlined in the Program Description If a Scholar has a lapse or break from the program for a total of more than two semesters (or three quarters) 19

Can Scholars be discontinued from the program? Fails to provide a transcript from their school showing at least 12 hours of credit each semester (or quarter) If the Scholar remains an ACH employee after the Award Ceremony, but is later terminated for disciplinary reasons If the Student does not remain “in good standing” as an employee 20

Who pays for the Program? Funds come primarily from confidential, voluntary donations by the residents, resident clubs, staff and local businesses Although the goal may be to award $6,000 per eligible Scholar, lesser donations to the Fund may result in smaller rewards 21
