Plan For Change By Group 5

Identified problem: Obesity Ineffective Health Maintenance The people of Grand Traverse County have a lack of familiarity with neighborhood resources as evidenced by higher than average obesity rate, cardiac disease rates, diabetes mellitus disease, and malignant neoplasms.

Intervention  Improve access to health maintenance activities  Make sure patients are aware of offered activities and availability

Community agency in charge of project Rationale: The mission of Munson Healthcare is to both “provide superior quality care and to promote community health" (Munson 2008). Munson Medical Center

Outcome Measures Some expected outcomes may include : Client verbalizes two measures necessary to achieve weight reduction Client demonstrates appropriate selection of healthy meals or menu planning toward the goal of weight reduction Client begins an appropriate program of exercise by choosing two activities to perform at least three times per week for six weeks Client is able to make positive dietary changes as evidences by utilization of available resources

Food Pyramid The client will list the six food groups of the food pyramid and describe the correlation between food and exercise.

The client will have fun while learning about nutrition by participating in learning activities.

Activity is important to maintain a healthy weight Studies show that lack of physical activity during childhood may lead to health problems in adulthood like obesity, heart disease, and Diabetes type 2.

Maintaining weight - Your weight will stay the same when the calories you eat and drink equal the calories you burn. Losing Weight - You will lose weight when the calories you eat and drink are less than the calories you burn Gaining Weight - You will gain weight when the calories you eat and drink are greater than the calories you burn.

Abstract Obesity is a health problem that the United States has struggled with for decades, and there are many health problems associated with obesity. The obesity epidemic is decreasing quality of life for millions and driving up the cost of healthcare. Obesity is a modifiable risk factor, and when people adopt healthy lifestyles and lose weight, they can reverse many detrimental health problems. By improving access to health maintenance activities and providing nutrition counseling on portion sizes and food label reading, clients will have improved knowledge to increase healthy behaviors and promote weight loss with an eventual decrease in risk factors related to obesity
