4 The key objectives for the Strategic Clinical Networks are to: Provide advice to commissioner Reduce variations Support the delivery of quality improvement Promote and disseminate good practice Develop mechanisms for patient and user experience (working through patient groups, the third sector and statutory parts of the system such as Healthwatch)

Strategic Clinical Networks Four networks have been mandated nationally: Cardiovascular (cardiac, stroke, renal and diabetes) Cancer Mental Health, Dementia and Neurological Conditions Maternity and Children In addition we have hosted Respiratory

Where are we now Developing case for change for interventional cardiology and stroke Maternity dashboard which has been adopted across the South. Supporting CCGs to improve diagnosis rates for dementia Supporting the cancer wait ambitions, an early diagnosis programme for cancer and supporting acute oncology configuration, Developing model of care for CAMHS to support Tier 4 services. “First fit” clinic agreement and alignment of ambulance pathways 6

Leading on 4 transformation programmes Parity of esteem – improving cardiovascular health for service users with Serious Mental Illness (SMI) Developing a single commissionable pathway for rehab with CCGs and specialised commissioning. Increasing the number of people who are able to die in place of choice and implementing the revision of the Liverpool Care Pathway (LCP). Supporting the introduction of Integrated Personal Commissioning Supporting the emergent urgent and emergency care review and developing blueprint for transition 7
