Medical Care is just part of our lives

Medical Home- definition Medical Goals Accessible Family Centered Continuous Comprehensive Coordinated Compassionate Culturally Effective What does it mean for patients Be there when I need you Recognize that I’m the most important member of the team Be my partner over time Provide or help me with the health care services I need Help me navigate the health care system. Know me as a person Speak to me so I can understand you.

Doctors’ offices are Changing YOU MAY FIND: “AFTER HOUR” APPOINTMENTS CARE COORDINATORS INFORMATION ABOUT COMMUNITY RESOURCES INFORMATION IN YOUR OWN LANGUAGE

Medical home care coordination Practice-Based Care Coordinators Not always done by the physician Makes sure specialists and primary care doctors are communicating Coordinates appointments Is aware of, and shares with you community resources Communicates with schools, mental health, dental Refers you to other families for peer to peer support

Doctor’s offices are changing You may experience: More questions about your child’s behavior More questions about your social living situations More questions about mental health More “screening” questionnaires about your child’s development More questions about your child’s school experience

Your level of participation Family Centered Care requires family input! Moving from “They told me…” to “WE decided…” Expect to share with your doctor what your child does well (his strengths) Expect to share your concerns Expect to participate in decision making and planning care and treatment Be prepared before attending a well child visit!

GOAL: PARENTS SHOULD FEEL MORE CONFIDENT CARING FOR THEIR CHILDREN WITH SPECIAL NEEDS AFTER AN APPOINTMENT, NOT LESS CONFIDENT!
