SIM Grant Update State Innovation Models Round Two 2 Reaching across Arizona to provide comprehensive quality health care for those in need

SIM Purpose 3 Reaching across Arizona to provide comprehensive quality health care for those in need SIM is based on the premise that state innovation with broad stakeholder input and engagement, including multi-payer models, will accelerate delivery system transformation to provide better care at lower costs.

4 SIM Purpose Reaching across Arizona to provide comprehensive quality health care for those in need SIM is focused on public and private sector collaboration to transform the state’s delivery system.

5 SIM Purpose Reaching across Arizona to provide comprehensive quality health care for those in need SIM provides financial and technical support to states to test the ability of state governments to use their regulatory and policy levers to accelerate health transformation.

Focus of the SIM Model Design Develop a State Health System Innovation Plan 6 Reaching across Arizona to provide comprehensive quality health care for those in need

The Arizona SIM Model Design Proposal 10 Reaching across Arizona to provide comprehensive quality health care for those in need

1. Behavioral Health- Physical Health Integration 11 Reaching across Arizona to provide comprehensive quality health care for those in need

2. AHCCCS, Medicare, Commercial- Value Based Care Delivery/Payment Models 12 Reaching across Arizona to provide comprehensive quality health care for those in need

3. Develop regionally based care coordination models for the AIHP including information sharing capacity and collaboration with IHS, 638 facilities, and non- tribal providers 13 Reaching across Arizona to provide comprehensive quality health care for those in need

4. Improve Justice System Transitions through development of HIT infrastructure and health plan interfaces to coordinate coverage and care 14 Reaching across Arizona to provide comprehensive quality health care for those in need
