SID Patient Interface An Update

Current SID System Interface is built on existing SID platform

The Process Identification of key stakeholder groups including clinical staff, voluntary and council employees, users of key LTC services Conducted interviews and information gathering process Analysis and extraction of key elements of design Revisited key groups or individuals to discuss and build on original findings The Process Identification of key stakeholder groups including clinical staff, voluntary and council employees, users of key LTC services Conducted interviews and information gathering process Analysis and extraction of key elements of design Revisited key groups or individuals to discuss and build on original findings Consultation Process Outcomes Clear set of objectives relating to what the interface needs to achieve Key design features that support both the individual and organisational requirements Failings in existing systems in relation to presentation of information and methods of access Do’s and don’ts Outcomes Clear set of objectives relating to what the interface needs to achieve Key design features that support both the individual and organisational requirements Failings in existing systems in relation to presentation of information and methods of access Do’s and don’ts

Key Outcomes Some Of What We Learned Access to genuinely local information is difficult The information and interface design needs to support the individual in their current situation The interface needs not only support self-management but should also educate about self-management Needs to work in unison with other sites such as My Condition, My Life and NHS Inform Be targeted towards specific points along the patient journey, with the information tailored to that point Help not just with the condition but with the practical issues that surround things like accessing services, being aware of patient responsibilities Interface needs to help the individual to make good decisions Should be designed around the genuine needs of those with Long Term Conditions in terms of management, diagnosis and relapse Locally SID strong brand for this type of site Some Of What We Learned Access to genuinely local information is difficult The information and interface design needs to support the individual in their current situation The interface needs not only support self-management but should also educate about self-management Needs to work in unison with other sites such as My Condition, My Life and NHS Inform Be targeted towards specific points along the patient journey, with the information tailored to that point Help not just with the condition but with the practical issues that surround things like accessing services, being aware of patient responsibilities Interface needs to help the individual to make good decisions Should be designed around the genuine needs of those with Long Term Conditions in terms of management, diagnosis and relapse Locally SID strong brand for this type of site

Initial Steps Development of Conceptual Model Derived from objectives and consultation Focused on architecture and flow of site Identifies key structures and their requirements Defines data structure and it’s presentation/interaction within the interface Development of Conceptual Model Derived from objectives and consultation Focused on architecture and flow of site Identifies key structures and their requirements Defines data structure and it’s presentation/interaction within the interface Feasibility Analysis Can it be built in terms of  Data requirements  Structure  Usability  Scale and size  Within Timescale Feasibility Analysis Can it be built in terms of  Data requirements  Structure  Usability  Scale and size  Within Timescale Improvements to Existing System Expanded existing database structure Improved and more efficient maintenance system SID Web Spider Improved layout Improved search functions Better system stability Improvements to Existing System Expanded existing database structure Improved and more efficient maintenance system SID Web Spider Improved layout Improved search functions Better system stability

Key Design Elements Strands Based on activity within 4 strands including: Physical, Emotion, Lifestyle, Social Strands are dynamic in nature and generated from within the database Strands are created by user through interaction in the interface Strands vary depending on variability of patient situation and experience i.e. condition Strands = routes through the interface Strands Based on activity within 4 strands including: Physical, Emotion, Lifestyle, Social Strands are dynamic in nature and generated from within the database Strands are created by user through interaction in the interface Strands vary depending on variability of patient situation and experience i.e. condition Strands = routes through the interface Interaction Points Combination of strands at different points creates complete dataset Range of resources and messages are applied at these points Interaction Points Combination of strands at different points creates complete dataset Range of resources and messages are applied at these points Prompts Messages and triggers presented throughout the interface Based around: user lead activity, organisational prompt e.g. appointment, events i.e. relapse in condition Designed to educate, guide and reassure Prompts Messages and triggers presented throughout the interface Based around: user lead activity, organisational prompt e.g. appointment, events i.e. relapse in condition Designed to educate, guide and reassure

Desired Outcomes For Users Understanding effects on you, across three strands Understanding the effects on others Better prepared for appointments and increase awareness of service requirements Understanding the things they need to do Greater understanding in preventing or dealing with relapse Improved quality of life Greater understanding of self- management Supports to self-manage Raise awareness of existing support in local community For Users Understanding effects on you, across three strands Understanding the effects on others Better prepared for appointments and increase awareness of service requirements Understanding the things they need to do Greater understanding in preventing or dealing with relapse Improved quality of life Greater understanding of self- management Supports to self-manage Raise awareness of existing support in local community The System Genuine support tool for self-management Accessible to all Provides the right information at the right point for the user Creates a bubble of resource and reassurance around the user Not just for patients but those that care for the individual The System Genuine support tool for self-management Accessible to all Provides the right information at the right point for the user Creates a bubble of resource and reassurance around the user Not just for patients but those that care for the individual
