Study Objectives Respond to the request by Independent Living Specialists for more knowledge in health promotion. Design, develop, and assess the impact of a Health Navigator intervention that Independent Living Specialists can use to equip consumers with skills and knowledge to manage their own health. Independent Living Specialists help consumers to obtain needed services and benefits to live in the community, while adhering to the independent living philosophy of people with disabilities making informed choices about how they will live. Health Navigators assist underserved patients with a person- centered approach to overcoming barriers to quality health care.

Study Design Develop and test curriculum with Consumer Empowered Team Content focus: persons with physical disabilities; chronic conditions and clinical preventive services; disability specific issues and barriers Expected Outcomes – After receiving health navigator training, ILS will value the content they have learned and will incorporate what they have learned into the services they provide to consumers. – Consumers receiving health navigation services from their ILS will improve their level of patient activation, health promotion, and community participation.
