Phyllis Tuckwell Hospice Grand Draw 2014 Terms & Conditions 1.By entering into the Phyllis Tuckwell Hospice 35 th Anniversary Grand Draw 2014 raffle, entrants agree to be bound by these terms and conditions. 2.Participation in the Phyllis Tuckwell Hospice 35th Anniversary Grand Draw 2014 raffle costs £1 per raffle ticket. 3.There are no restrictions to the number of entries one person can make. However, the operator has a responsibility of care towards its players, to identify any signs of potential irresponsible gambling and raise the issue accordingly. 4.Monies raised by the Phyllis Tuckwell Hospice 35th Anniversary Grand Draw 2014 raffle will go towards the work of Phyllis Tuckwell Hospice to provide Specialist Palliative Care. 5.Payment for participation in the raffle can be made by cheque (made payable to Phyllis Tuckwell Hospice) credit card or cash. 6.The ticket is in two parts. The left-hand, smaller section of the perforated ticket, known as the ‘counterfoil’, is to be returned and physically entered into the draw. The right-hand, larger section of the ticket, known as the ‘ticket’, must be kept by the player and produced to claim any related prizes. 7.Your raffle tickets (ticket ‘counterfoils’) will be entered into the draw as soon as the whole payment is received. 8.The promoter accepts no responsibility if the chosen method of payment is cancelled in error. 9.No responsibility is taken for raffle tickets and payment lost, delayed or damaged in the post and proof of posting will not be accepted as proof of receipt. 10.Closing date for the receipt of raffle ticket counterfoils and raffle ticket payment by cheque or credit card in the post to the Hospice is Monday 20th October Tickets received after this date in the post may not be entered in the draw. Tickets purchased in cash can be bought from and delivered to the Hospice by hand and can be received into the raffle until one hour before draw time. 11.Any tickets received after the above dates and times, and deemed too late to be entered into the draw, will be retained, and the monies returned if full postal address details are legible on the counterfoil. 12.The Phyllis Tuckwell Hospice 35th Anniversary Grand Draw 2014 raffle will take place on Monday 27th October 2014 at Phyllis Tuckwell Hospice at 3pm. Entrants do not need to be present at the draw to be eligible to win a prize. 13.Phyllis Tuckwell Hospice reserves the right to change the date and/or time of the draw. 14.Raffle tickets will be drawn randomly. The first ticket number drawn will receive the first prize of £2,000 cash. The second two ticket numbers drawn will receive a cash prize of £500 each. The third two ticket numbers drawn will receive a cash prize of £100 each. The next 20 ticket numbers drawn will receive a cash prize of £50 each.

Phyllis Tuckwell Hospice Super Draw 2013 Terms & Conditions 15.Tickets and payment received by 5pm on Friday 25 July 2014 will be entered into an Early Prize Draw for the chance to win £100 M&S Vouchers. One raffle ticket will be drawn at random on Monday 28th July 2014 at Phyllis Tuckwell Hospice at 3pm. 16.Tickets are non-transferable. 17.The promoter reserves the right to change the details and availability of any of the advertised and listed prizes at any time. 18.Winners will be notified by telephone and/or post. Any prize not claimed after a period of three months from the date of the draw will revert automatically to Phyllis Tuckwell Hospice. 19.Phyllis Tuckwell Hospice staff are eligible to take part in the raffle unless they are the named promoters, Fundraising or Finance. Ineligibility extends to the immediate families of the above. 20.All entrants are solely responsible for providing Phyllis Tuckwell Hospice with their accurate and up-to- date contact details and Phyllis Tuckwell Hospice will be in no way liable for any failure or inability to contact any entrant due to any errors, omissions, illegibility or inaccuracies in the contact details that the entrant has provided. In the event that an entrant changes their contact details, they will be solely responsible for advising Phyllis Tuckwell Hospice of the change. 21.By accepting a prize, the winners agree to take part in any promotional activity deemed appropriate by Phyllis Tuckwell Hospice. Phyllis Tuckwell Hospice reserves the right to use the name and first line of address of the winners, their photograph and audio and/or visual recordings of them in any publicity unless prior notification has been received. 22.The result of the 35th Anniversary Grand Draw 2014 raffle draw is final. No correspondence will be entered into. 23.The draw will be overseen by the promoter/operator and in the presence of two independent witnesses. 24.The Phyllis Tuckwell Hospice 35th Anniversary Super Draw 2014 raffle is open to all mainland Great Britain residents over the age of 16 (this excludes Northern Ireland, Isle of Man and Channel Islands). The tickets may not be sold to, or by, or for, a person under 16 years of age. 25.Nothing within these terms and conditions shall create or should be construed as creating any form of contract between any entrant and Phyllis Tuckwell Hospice. 26.Any complaints relating to the 35th Anniversary Grand Draw 2014 raffle should be sent in writing to the Head of Fundraising at Phyllis Tuckwell Hospice giving full details of the complaint and any relevant supporting documentation.

Phyllis Tuckwell Hospice Super Draw 2013 Terms & Conditions 27.Phyllis Tuckwell Hospice is committed to protecting the entrant’s privacy. Data that is collected from the entrant is used lawfully in accordance with the Data Protection Act 1998 and is used for the purpose of processing 35th Anniversary Grand Draw 2014 raffle tickets and informing winners that they have won a prize. Additional personal details of existing records (for example, mobile number and ) will be added to the Hospice’s database unless the entrant has ticked the data protection statement on the ticket stub. New records will be created for all entrants not previously known to the Hospice who have not ticked the data protection statement. These details will be used to inform individuals about the work of the Hospice and its promotional activities. 28.Any entrant has the right to access the information held about them. To obtain this information, please contact Phyllis Tuckwell Hospice in writing. 29.Phyllis Tuckwell Hospice will not sell, rent or grant access of any personal data to any third party without express prior permission. 30.Phyllis Tuckwell Hospice reserves the right to amend these rules at any time. If amendments are made, they will be published on the website at 31.Phyllis Tuckwell Hospice reserves the right to disqualify any entrant if it has reasonable grounds to believe the entrant has breached any of these rules. 32.Each entrant should retain a copy of these terms and conditions for their reference. 33.The 35th Anniversary Grand Draw 2014 raffle is a form of gambling.
