1. Start-up Activities  HUA IRB approval required?  Other permissions required?  Informed consent?  Incentives?  Manual of procedures (MOP)/ documentation?  Project website?  Timeline?  Potential roadblocks?

2. Training  Who will conduct the training?  Who will be trained?  Role and timing of student researchers?  When?  What?  How to conduct focus groups, interviews, household surveys?  How to enter data?  Resources needed?

3. Focus groups and in-depth interviews  Who to invite, types of respondents, in what order?  Sample size?  Local customs – provide food?  What questions to ask?  Who will collect data?  Audiorecording and transcribing?  Translation?

4a. Household survey: Sample selection and recruitment  Inclusion/exclusion criteria?  Advance notice of timing, content, methods?  Participation rate?

4b. Household survey: Questionnaire development  Length?  What questions in what order?  Translation and back translation by whom?  Pre-test (N=12?) in each zone x setting?  Examine pre-test data and interviewer feedback and revise questionnaire and MOP

4c. Household survey: Data collection  How many interviewers available, from where?  Over what time period?  PDA (Vietnamese software available?) vs. paper questionnaire?

4d. Household survey: Data entry and management  Double data entry (enter and verify)  Confidentiality procedures?  Data transfer procedures to Ian?

4e. Household survey: Data Analysis  Develop analysis plan  Documentation of development of indices, recodings, etc  Document revisions to analysis plan  Document location of analysis files
