Pre-operative Case Management

Topics Phases of Surgery –Preoperative –Intraoperative –Postoperative Pre-operative Case Management Preference Card Utilization Equipment Selection

Phases of Surgery Pre-operative –Prior to initiation of surgical procedure –Begins with the skin incision or endoscope insertion Intra-operative –Time frame between skin incision or endoscope insertion and ends when the surgical dressing has been applied or the endoscope has been removed Post-operative –From application of surgical dressing or removal of the endoscope until the patient has been discharged from PACU

Pre-operative Phase of Surgery Donning OR attire and PPE Preparing OR Gathering supplies, instrumentation, equipment Opening of sterile field supplies and items Surgical hand scrub Toweling, gowning and gloving Preparation of sterile field Initial pre-operative count VS monitoring Transfer of patient to OR bed Positioning patient Assisting other sterile team members Prepping the patient Draping the patient Pulling up to the field and passing off cords, tubing, light handles

Principles of Asepsis “A sterile field is created for each surgical procedure” “Sterile team members must be attired appropriately before entering the sterile field” “Movement in and around the sterile field must not compromise that sterile field” (Price. ST for the ST, 2 nd ed., p. 319.)

Principles of Asepsis The principles of asepsis have concepts or rules for each of the three of them that must be observed at all times These rules are the key to the practice of sterile technique The application of these principles and each of their rules is referred to as Principles of Asepsis Adhering to the principles is governed by the surgical technologist’s surgical conscience which entails a moral integrity to provide safe care to the patient at all times “Aeger Primo” Patient First

Principles of Asepsis Text ST for the ST pp the Principles of Asepsis are reviewed and explained there You are responsible for knowing and practicing these at all times as a surgical technologist

Textbook Reading Assignment You are responsible for reading the following for this section Pre-operative Case Management pp Skills assessments will be done for the skills discussed in this chapter in lab You are responsible for understanding why we are doing the skills the way we are doing them will explain this in lab

Concepts to Review/Read/Know Irreducible minimum Violating the sterile field –Options Standard precautions Critical thinking –APOS Anticipation Communication and Teamwork in the OR
