Getting Started Please have the following information before starting: Health center grant number or H80 number Full name and address of the health center and BHCMIS ID located in box 9 on Notice of Award (NoA) BPS site ID number, name and address of each site applying for recognition Name of your HRSA project officer Alternate contact person and contact information for the recognition process 6

Things to Remember A Notice of Intent (NOI) is necessary to participate in HRSA’s Accreditation and PCMH Recognition Initiative Allow 2-3 weeks for HRSA to complete an internal review for organizational readiness Effective March 1, NOI will be processed through the new system Step by Step instructions will be available New mailbox: BPHC Helpline
