2014 Girls’ Future Tiger Camps Grade 3 – 6 Session I August 4 th – 7 th 8:00 – 9:45 Valley High School Grade 7 – 8 Session II August 4 th – 7 th 10:00 – Valley High School Parent’s Release and Indemnity Agreement In consideration of the acceptance of this application for enrollment in the 2011 Tiger Basketball Camp, I intend to be legally bound and release any and all rights and claims for damages I may have against West Des Moines Community School, all sponsors and all employees of the 2011 Tiger Basketball Camp for any damages which may be sustained and suffered in connection with participating in camp. I hereby grant permission for my child to be a participant in the 2011 Tiger Basketball Camp and if any injury should occur during traveling to, participating in or returning from camp, I agree to pay for all costs, present and future, through my medical insurance policy and/or personal finances. Camp Features Future Tiger Basketball camp will provide a fun atmosphere for campers to learn the fundamentals of basketball. They will receive instruction from Valley High School coaches and players on offensive and defensive drills with an emphasis on improving individual skills. Campers will participate in games, drills, and friendly competitions throughout the week. Recognition and prizes will be awarded for skill level, hard work, and improvement. All campers will be placed into groups according to their age and ability. Age groups will be determined by the grade campers are going into in the Fall of Camp Fees and Registration Information Camp Fees Individual fee is $60. Family of two is $110. Family of three is $140. Check – in Will be 30 minutes before the start time of the camp session. Registration Deadlines Session I & II Camp registrations will be accepted by mail until July 30 th.
