Skills in note-making, summarising and report writing Quality and Outcomes Framework Assessor Training

To understand the importance of making accurate recordings To be able to make accurate and relevant notes To be able to write well constructed reports Aims of the Module

If practices believe they have been badly treated they will challenge the accuracy of your report You cannot return later if you discover you cannot understand your notes Once your report has gone off it is very difficult to correct mistakes Two key points

It is extremely difficult to get an accurate picture of a practice in a short space of time Misconceptions and mistakes are all too common Nothing creates a bad impression more than if a report is sloppy or inaccurate Remember also..

Making notes throughout your visit is essential Write down what you hear or see – not your own interpretation Ask to see the evidence – notes, registers etc. Flag up key points in the margin Making notes 1

Summarise what you think you heard by reflecting it back If you subsequently cant understand what youve written go back and check it out Check that you have identified all key areas against the new contract documentation Making notes 2

Decide at the beginning of the visit who will write the report Write in clear English and state evidence not your impressions Follow a clear template to ensure everything is covered Report writing

Title, place and date Names of assessors People seen Findings in sections Conclusions, including strengths and recommendations Report – suggested format

Good report writing is rare Many have poor or nonexistent evidence Many have sloppy English If the practice doesnt like your conclusions they will go through your report with a toothcomb Get it right! Conclusion
