Clinica’s Mission & Vision Clinica is a community health center that provides affordable health care to low-income, uninsured people living in southern Boulder, Broomfield and western Adams counties. Our Mission: To be the medical and dental care provider of choice for low-income and underserved people in south Boulder, Broomfield and west Adams counties. Our care shall be culturally appropriate and prevention focused. Our Vision: Every low income and underserved person in south Boulder, Broomfield and west Adams counties will have access to high quality, preventative medical and dental care that is integrated with behavioral health care.

Our Past Our First Clinic – Lafayette, CO  Founded by Alicia Sanchez – single mother of 7 children, no high school education, living with Lupus  Opened 1977 – During first year, saw 500 patients on a budget of $75,000 with six employees  Patient Population in First Year – Migrant farm workers, low- income locals, predominantly people needing acute care, adults with high blood pressure and pregnant women.

Today! Four medical clinics Two dental clinics An administrative building 38,450 patients in 2010 using 181,665 appointments Current budget: $27,775, employees Two full-service pharmacies

Clinica’s Service Area -- Boulder, Broomfield and Adams Counties, Colorado

Within Our Service Area… 628,007 residents (1 in 10 Coloradans) 150,681 low-income individuals (living on less than $44,100 annually for a family of four) 81,641 uninsured individuals

Clinica’s Patient Demographics ,092 patients (44%) 18 and younger 10,067 women (26%) of child-bearing age (20-44 yrs) 1,286 total patients (3%) over the age of 65 97% patients living at or below 200% poverty level 46% uninsured 41% Medicaid eligible

Need for Clinica’s Services While 150,681 individuals in our service area are eligible for our services, we currently have capacity to provide care to only 26% of those in need. Where do the others go? Some are young & healthy Area urgent care facilities Hospital emergency departments Nowhere (they go without the care they need)

Hospital Care Physicians on staff at three hospitals: Avista Adventist Hospital St. Anthony North Hospital Boulder Community Hospital (no OB)

Care for the Homeless Provided care to 3,201 homeless patients in 2010 Full-time homeless outreach nurse On-site care at day shelter, overnight homeless shelter, churches that serve dinner In conjunction with hospital and homeless shelter, developed a nursing respite program for homeless post- hospitalization
