Welcome to Sports Information Night “Individual commitment to a group effort - that is what makes a team work, a company work, a society work, a civilization work.” Vince Lombardi Our Goal is to develop Character, Leadership, Dedication, Perseverance, Discipline, and Hard Work to become part of something greater than themselves.

DHS Sports Information Night Agenda Academic eligibility a. Fall Season = in coming 9th graders automatically eligible. b. Winter Season = 2.0 GPA and not more than 1 E for the first quarter. c. Spring Season = 2.0 GPA and not more than 1 E for the second quarter. d. 4th quarter marking period will determine eligibility for the following Fall Season. e. You must maintain a 2.0 and not more than 1 E during the season as well. e. You can monitor grades from Pinnacle on your child’s Edline account. ECA fee $30 (Extra Curricular Fee) = you can pay by credit card online or by check with the forms that are on the Athletic page of our schools website.

Forms needed before the start of the season: 1)Health Inventory (Physical)- The form is available on the athletic page of the Damascus High School Website or on the door of the Athletic Office. I recommend that you get the Heath Inventory done in the months of June or July ( this avoids problems with not being able to get an appointment before tryouts). Make 3 copies of your health inventory, you will need to present a copy to the coach of each season you are trying out (we will no longer except “we have one on file”, if you do not have a copy of the health inventory, your child will not be able to try out). Do not turn the form into the Main Office or Athletic Office

Parent Permission/ Residency/Student Agreement form: (will be available on the Athletic Page of the DHS website in late June) This includes: The parents permission to participate in that sport. Verification that you live within the Damascus Cluster (if not you must get a waiver from Central Office). Indication of whether or not you have insurance (Football players must have insurance- either their own or the one offered by the school system-these forms will be on the door of the Athletic Office before the fall season). Drug and Alcohol Agreement. The student agrees to refrain from using drugs and alcohol, parent agrees to support it. If you are caught using Drugs and/or Alcohol, you are suspended from the Team for 10 days. If you are caught using Drugs or Alcohol out of season, the suspension will be the first 10 days of the next season of the team you play for. Second Offense, you are suspended for one full year. Make 3 copies of the form (1 for each season-just incase you decide to play more than 1 season). Turn the form into the coach by the first day of tryouts (Do not turn the form into the Main Office or Athletic Office)

Medical Card ( Athletic page of the DHS website) Parents fill out the card to indicate what actions should be taken in case of an emergency (what hospital to take them to, emergency phone numbers) Other pertinent information (medical conditions, allergies, etc.) Coach keeps this in the Medical Kit which travels with them at all times (coaches are certified in CPR/AED) Make 3 copies of the form. Turn one copy into the coach by the first day of tryouts ( do not drop it off at the Main Office or Athletic Office).

Are you ready to join the Damascus High School Athletic Program? a)Commitment – Can I attend practice everyday for 2 hours and give 100%? Can I do the same for all the contests- am I taking up a spot on the team from someone who is committed? b)Responsibility – Maintain a 2.0 GPA or higher with no more than one E. Refrain from using drugs and alcohol, you represent your team and your school at all times. I cannot let them down. You impact the team if you are suspended - no one to blame but yourself. Return all borrowed equipment/uniforms immediately after the season is over. Participate in all Team fundraisers and help anyway you can. c)Follow Through- Return all forms to the head coach on or before the first day of Practice (Fall is August 15 th, Winter is November 15 th, and Spring is March 1 st ). You need to have a current physical (we recommend you get one in June or July- Providers sometime make you wait over a year to get another one). We have a Physicals Day here at DHS on June 6 th, $40 fee will benefit the Football program and the DHS Athletic Department (See our website and click on Athletics)

Athletic Booster Club Link on the Athletic Page of the DHS Website Without there support we would not function. The Boosters Club sponsored fundraisers supply approximately 50% of our financial needs. Please consider Joining the Boosters Club and get involved. We find student athletes have : a. better grades (not many athletes on ineligibility list) b. less behavior problems and administrative referrals c. are better role models- build strong character and leadership skills
