Patient Identification Always use Two Patient Identifiers: Name Date of Birth History Number Inpatients: Check Name and History Number on patient’s wrist band with the label. Outpatients: Patient states Name and Date of Birth while you compare Labels and Requisition. Always label containers (Blood, Urine, etc.) in the presence of the patient. Never use the patient’s room as a patient identifier.
