What is Graduation Transitions? Graduation Transitions is a mandatory course for all BC Grade 12 students on the 2004 Graduation Program.

The Main Components: Personal Health Increasing the knowledge, attitudes and positive habits needed to be healthy individuals Community Connections Learning the skills required to work effectively and safely with others Career and Life Developing the confidence and competency needed to be self-directed individuals

Personal Health  To complete this section of Graduation Transitions you must…  document 80 hours of physical activity in addition to PE 10  indicate how your physical activity aligns with your personal fitness goals, while considering your lifestyle and physical limitations.  How do you do that?  Download and fill out the Physical Activity form from the Grad Transitions page of the CIDES website.

Community Connections  To complete this section of Graduation Transitions you must…  document 30 hours of work and/or volunteer experience  describe the duties performed, the connections between the experience and employability/life skills, and the benefit to the community  How do you do that?  Download and complete the Work Experience form from the Grad Transitions page on the CIDES website. Include this with your final presentation.

Career and Life  In this section of the Graduation Transitions, you are required to make up a Transition Plan and arrange an Exit Interview.  The Transition Plan is intended to help you move from high school to into the phase of your life.

Transition Plan  Components the Transition Plan?  resume  covering letter  evidence of career and/or post secondary school research  evidence of financial planning for your first year out of high school  a completed application form -- for a work or school opportunity

What should the Transition Plan look like?  A Transition Plan can take many forms:  Paper-based  Binder  Portfolio  Electronic  PowerPoint  Webpage  Show a prospective employer, school or organization who you are and what you can do.  The idea is to make it look professional -- but don’t be afraid to be creative.

Getting help with a Transition Plan  Get feedback on your Transition Plan from family, friends, teachers, etc. Incorporate their comments and ideas into your work.  Each part of the Transition Plan must be reviewed by the teacher, Mr. Olexyn. Use , phone contact or appointments to keep him up to date on new developments  You must bring your Transition Plan to your Exit Interview. The interviewer may want to ask you specific questions about it.

Exit Interview  The Exit Interview is a brief discussion of your Transition Plan and future goals and plans with a staff/community member.  The Exit Interview will take approximately minutes.  Book your Exit Interview by phone or with Mr. Olexyn when you are ready.

Celebrating You and Looking Forward to the Future  Graduation Transitions is meant to be a celebration of your achievements in high school and a way to ready yourself for life beyond secondary school.  We look forward to seeing your completed material and helping you prepare for the next stage of your life.
