What is a Personal Health Budget? “A personal health budget is an amount of money to support a person’s identified health and wellbeing needs and outcomes, planned and agreed by the service user and their local healthcare professional.” Its not just about money, it is the personalised care and support plan that is central to a personal health budget.

Finances

Who can have a Personal Health Budget? 1 October 2014, ‘right to have’ for adults, children and young people eligible for NHS Continuing Healthcare. 1 April 2015, ‘right to ask’ for those with long term conditions, including mental health conditions, who could benefit.

Bristol CCG Progress Currently implemented/progressing 14 CHC adult PHBs and 5 CHC children/young person PHBs. Trialled one off PHBs for 1 mental health and 1 individual funding request service user. Developed processes and policy. Learning curve and continuing to learn and develop with each case.

Why? Bristol CCG CHC Personal Health Budget Holder ‘Having a Personal Health Budget not only gives me the freedom to source care whenever and however I wish [in line with my personalised care and support plan agreed with the NHS] but it has enabled me to plan a holiday for the first time in many years. The confidence and security it brings means that I am entering the next phase of my life with renewed purpose and optimism, with those close to me already noticing an improvement in my general health and well being’

Integrated Personal Commissioning Pilot programme April 2015 to March 2017 aimed at building on personalisation and integration agendas. Partnership between CCGs, Local Authorities and VCS. Programme aimed at: people with long term conditions, including frail elderly people at risk of care home admission; children with complex needs; people with learning disabilities; and people with severe and enduring mental health problems.

IPC Purpose Year of Care Model of person centred care planning for these cohorts. Develop a capitated budget financial model that brings together costs of primary, mental, community, social and acute care for individuals into one pot. The contract is on the basis of a price per person (capitation), based on level of need, e.g: –£X a year for someone with 1 LTC; –5X a year for someone who is over 75 with dementia and 2 other LTCs. Option for personal budget (integrated health and social care where appropriate)

Opportunities for the voluntary and community sector Co-designing the development of PHBs and the IPC programme with CCGs and Local Authorities. Provide advocacy and brokerage support for individuals who want a PHB or integrated budget. Assist with care and support planning development. Developing peer networks. Supporting Local Authorities and CCGs to genuinely co-produce new approach and support those groups who are less often heard. Voluntary sector initiatives, such as social prescribing, have also shown the impact of VCS provision in building community capacity and providing alternative approaches that boost the health and care economy.
