Purpose To evaluate the efficacy of cataract surgery with presbyopic intraocular lens (IOL) insertion in mild/moderate glaucomatous versus non-glaucomatous eyes.

Methods Retrospective, single center, multi-surgeon study including 55 eyes of 39 cataract surgery patients with or without mild/moderate glaucoma receiving a presbyopic IOL (Restor D1 & D3, Crystalens HD & AT50SE, or Tecnis Multifocal) between 2005 and glaucoma patients (24 eyes; mean age 76.4y; 42.9% male) Pre-Op VF Data: Avg. MD = -3.5; Range = to Lenses: Restor (n=20), Crystalens (n=3), Tecnis (n=1) 18 non-glaucomatous control patients (31 eyes, mean age 67.6y, 44.4% males) Lenses: Restor (n=21), Crystalens (n=4), Tecnis (n=6) UCVA, BCVA, and near acuity were measured one month following IOL insertion. Post-operative enhancement procedures were recorded. No patients required IOL removal. One glaucomatous patient required excimer laser treatment for residual refractive error.

Results % of eyes achieving a post-op UCVA of 20/25 or better was comparable: 47.8% of glaucomatous and 45.2% of control eyes. % of eyes achieving a post-op UCVA of 20/40 or better was also similar: 91.3% of glaucomatous and 83.9% of control eyes % of eyes achieving a post-op BCVA of 20/25 or better was comparable: 65.0% of glaucomatous eyes and 75.0% of control eyes % of eyes achieving a post-op BCVA of 20/40 or better was equal: 100% of glaucomatous eyes and 100% of control eyes % of eyes achieving an uncorrected near visual acuity of J3 or better was not significantly different: 79.2% of glaucomatous eyes and 92.6% of control eyes

Conclusions Patients with mild/moderate glaucoma receiving presbyopic IOLs obtained distance and near visual outcomes comparable to non-glaucomatous patients receiving similar presbyopic IOLs. Our results indicate that glaucoma patients may benefit from presbyopic IOL insertion.
