Pregnancy Rates After Breast Cancer Available data suggest a ~ 5-15% pregnancy rate after breast cancer A survey study showed that 57% of 60 young women trying to conceive after treatment were successful It is unknown whether women who continue to menstruate after chemotherapy remain as fertile as they might have been had they not undergone chemotherapy. There are few data on actual fertility and pregnancy outcomes following chemotherapy in breast cancer survivors. Fox et al. followed a cohort of 24 premenopausal women with breast cancer for fertility outcomes following chemotherapy.14 All of the women, ranging in age from 23-42 years at diagnosis (median 35) received leuprolide, a gonadotrophin-releasing hormone (gnrh) analog, during chemotherapy, in an effort to preserve their ovarian function through chemotherapy. Within 12 months after chemotherapy, 23 out of 24 patients had resumed menstrual functioning, and 15 out of 20 reported regular menses at a mean follow-up of 34 months. However, at that time, there had been only 6 pregnancies in five patients, with three women requiring fertility treatments. Three of the pregnancies resulted in miscarriage, one was terminated for a Down’s Syndrome fetus, one resulted in a live birth, and one was ongoing at 8.5 months at the time of the report. Three additional patients reported unsuccessful attempts at conception despite fertility treatments. Fox et al., ASCO 2003; Partridge et al., ASCO 2004; Petrek, Personal Communication March 2005; Ives et al., BMJ 2007; Dalberg PLosMed 2006; *Partridge et al 2008

Contributors to Infertility In Cancer Survivors Need to delay conception until after time of most recurrences (> 2 years) Advanced maternal age Use of Tamoxifen x 5 years post chemo in premenopausal breast cancer MD and patient concern about effects of pregnancy on cancer recurrence Patients’ concerns that chemo may have mutated remaining sperm or oocytes, or that children may get cancer

Pregnancy After Breast Cancer There is no evidence that pregnancy increases the risk of breast cancer recurrence However women who do become pregnant may be in some way different than women who do not Oncologists may be good at determining who may safely become pregnant and who not It is more difficult to diagnose breast cancer in women who are pregnant Breasts are full and lumpy Mammography not as accurate
