Core Elements

 Recruitment Goals = 48 new participants each year  Recruitment for Intensive Activities 1. Recruitment Team 2. Team Meetings  Invitation to participate to those who meet eligibility requirements (prediabetes, age 18 or older, no exclusions)  Entry process - if individual agrees to participate  Documentation of Recruitment activities (for internal use and for reporting to IHS)  Settings for Recruitment  Methods of Recruiting individuals to participate  Materials for Recruitment

age> 18  Identify individuals at risk for diabetes age > 18  Screening and Diagnosis Settings 1. Community 2. Clinic  Diagnosis of prediabetes mg/dl 1. Fasting blood glucose mg/dl (Impaired Fasting Glucose, IFG) OR mg/dl 2. 2-hour glucose mg/dl (Impaired Glucose Tolerance, IGT) OR > 5.7 and 5.7 and < 6.5 (note: this remains an area of controversy for the diabetes community. Our best evidence suggests this cut-off at this time.)  Exclusion of Individuals  Pre-existing diagnosis of prediabetes  Monitoring/Follow-up of Participants with Prediabetes  Monitoring/Follow-up of Individuals Who Are Screened but Not Diagnosed with Prediabetes

 Diabetes Prevention Team 1. Team Membership 2. Team Meetings  Intensive Activities 1. Schedule appointment to start DPP Curriculum 2. Schedule Baseline Assessment  DPP Curriculum 1. Teach 16 session curriculum over 16 – 24 week 2. Goal – 48 newly enrolled participants each year 3. Recommend 12 participants per group, 4 groups per year 4. Group education sessions, with interactive sessions 5. Use DPP Curriculum/Lifestyle Core Classes as basic required information  Individual Coaching Sessions  After Core Contact  Lifestyle Goals for Individuals  Motivational Activities  Monitoring/Follow-up of Participants with Prediabetes  Documentation of Intensive Activities – Required Forms

 Retention Goal = 100% of your participants  Retention Plan 1. Retention Activities to Promote Continued Participation 2. Identification and Removal of Barriers to Participation  Tracking of Participation 1. Document attendance of participants in program activities 2. Recovery Efforts – when participant misses appointments or classes  Documentation of Retention
