Use and distribution limited solely to authorized personnel. © 2009 CIGNA 8 Transition of Care Benefits  Transition of care benefits are intended to allow members to continue to receive services for specified medical and/or behavioral conditions for a defined period of time with physicians who do not currently participate with CIGNA HealthCare. This will allow continued, uninterrupted care until the safe transfer of care to a participating physician or facility can be arranged.  Examples of acute medical conditions that may qualify for Transition of Care benefits: ­Pregnancy in the second or third trimester at the time of the effective date of coverage ­Newly diagnosed or relapsed cancer ­Trauma ­Recent major surgeries still in the global follow-up period  Enhanced Benefit for Ellington Employees: Behavioral Health claims will be automatically covered under transition of care for the first six months and you are not required to fill out any paper work, this will happen automatically.  If you have a medical condition that may apply you will need to fill out a TOC form with in 30 days of your effective date, July 1 st. TOC forms must be returned by July 31 st for review.  For approved medical acute conditions, you will have about 90 days or until a safe transfer can be made.

If you do not fill out this form you will not have a savings account for your funds to be deposited.  After you open your bank account through JP Morgan you will receive your new debit card and Welcome Packet in June.  You must go on-line to the below link and use the id provided to Ellington employees /welcome.do /welcome.do ID: EllingtonBOE2011

For example, adding or deleting dependents.  Forms are due in the business office by the end of the day May 13 th.  Effective July 1 st – be sure to show your new CIGNA ID Card when visiting your providers.
