The Underlying Factors of SCENAR Therapy: Effectiveness in View of SCENAR Technology by Iosif Semikatov Ekaterinburg 2014

 The living organism can exist in an optimum manner within a short range of the internal environment parameters.  The external environment changes these parameters. The organism consistently aims to compensate such changes in order to restore the optimum parameters of the activity. This is a homeostasis process.

 The homeostasis is realized due to formation of adaptive reactions which constitute a result of the activity of functional and adaptive systems in the body.  If there are any dysfunctions of these systems, any various diseases and states are formed and manifested as different complaints.  The official and alternative (holistic) medicine have different approaches to a disease treatment process.

TREATMENT APPROACHES:  Orthodox medicine – fighting against the disease.  Alternative medicine – harmonizing the patient’s body with the outside world and environment

Conventional (orthodox) medicine Alternative (holistic, systemic) medicine SCENAR Therapy – New Medical Technology Russia, Taganrog, RITM OKB ZAO The new medical area – SCENAR-therapy was formed at the junction of the official and alternative system approaches

SCENAR-therapy is the first medical technology where the treatment process (where, when and how long to treat) is controlled by the patient’s body rather than by the operator (specialist). Yu.V. Gorfinkel: “We must help the body help itself.” The core element of SCENAR technology is the BODY OF A PATIENT!

SCENAR-therapy has evidenced its high effectiveness in treatment of various diseases and states throughout its existence. This technology is applied in various areas of the medicine both in acute states or reanimation and in aggravation of chronic problems. SCENAR-therapy is used as a basic therapy and an accompanying therapy, or as combined with any other methods for the patients’ complex treatment.

Basic factors of high effectiveness of SCENAR-therapy : 1. Unique technical solutions. 2. Philosophic approach. 3. Medical aspects.

1. Unique technical solutions. - Application of optimum physiological parameters of an electric impulse. - Automatic correction of the stimulation stimulus parameters subject to the body’s reactions to a previous stimulus.

Specific characteristics of the SCENAR impulse The impulse pattern is similar to the impulse of the nerve fiber (catalyzing the production of regulatory peptides and prolonging effects). SCENAR impulseECG record

s ,2с t′(ms) J U ,2 s S TEMPORAL RELATION BETWEEN CURRENTS, ACTION POTENTIAL AND CHANGES IN EXCITABLE TISSUES  The impulse is very short-termed (µs). In traditional physiotherapy impulses are longer in duration (msec).  SCENAR impulses have long pauses between them to let receptors have rest. SCENAR therapy has no side effects of electrical treatment and does not cause damage to nerve fibers. 1.Diadynamic therapy 2.Amplipulse therapy 3.SCENAR-therapy 4.Action potential 5. Change in tissue excitability with excitation

1. Physiological compatibility: - the neural-like pattern (is perceived similarly to an impulse of the body; does not cause any negative side effects; is easily transmitted into the energo-informational paths, - bipolarity (alternating current does not cause polarization of tissues), - availability of pauses (giving time to the nerve fiber to have rest and to restore). 2. Nondestructive effect is achieved due to the short duration of the impulse ( µs). 3. Responsiveness, asymmetry (the impulse can change its shape and amplitude), thus significantly reducing the risk of habit formation. Specific characteristics of the SCENAR impulse

DYNAMIC INTERACTION SCENAR IMPULSE SKIN SKIN RECEPTORS CNS SKIN IMPEDANCE SCENAR IMPULSE Flow of information to regulation centers Flow of command information from the center to periphery ELECTRODES

2. Philosophic approach.  SCENAR-therapy is a philosophic concept with its own ideology relating to : - the world around us, - notions of the health and disease, - unity and interrelation of organs and systems of the organism as a whole. - the organism as a component of the environment.  The full active co-participation of a patient himself/herself in the recovery process, change of its attitude to the life (its way of life and thinking) to keep the result obtained.  Formation of a treatment process under SCENAR-therapy principles that essentially reflect the laws of the World.

 Each individual is a part of the World and reflects its structure in full.  The man constitutes something more than its body. This is a whole and mutually dependent system of the body, emotions, the brain and spirit.  The health keeping depends on this unity harmony.

Adaptation Time What is a disease? What is health? ? Adequate adjustment adaptive reaction (self-regulation) – health Adaptation Time I. Inadequate adjustment – disease Incomplete adjustment – chronic condition Stimulus Stimulus Stimulus

The following principles shall be observed in treatment to improve the effectiveness. Underlying principles and basic concepts of SCENAR therapy 1.“Here and now” Complaint 2.Zone and Vector (direction) 3.Asymmetry and Small Asymmetry 4.Dynamics and Stability 5.Opposite and Cycle 6.Combination and Alteration 7.Saving and Optimality The application of these principles helps to form a “therapeutic triangle“ and a medical algorithm of SCENAR-stimulation as a basis of SCENAR-technology effectiveness.

Control of the device Body (Patient) Operator (specialist) SCENAR device Multiparametric feedback Objective & Subjective feedback THERAPEUTIC TRIANGLE FEEDBACK

The medical algorithm includes : А. Stimulation of a planned skin area. The stimulation purpose is to expertise this area state and look for Asymmetries (areas with different reaction on the same device operation) on it. В. Selection of an area with the similar difference of the Small Asymmetry zone (a skin area of the smallest square and maximum intensity of manifestations) for additional stimulation of this area with SCENAR device.

С. Stimulation of the selected Small Asymmetry zone with SCENAR-device in order to obtain planned changes on the skin and in complaints. The stimulation purpose is to concentrate the counter irritation on the area of smaller square in order to form a vector of the body’s responses and obtain positive dynamics of the patient’s complaints.

3. Medical aspects.  The patient’s treatment process is formed as a dialogue (under the feedback principle). The methodology based on the body’s neurophysiology and formed taking into account its functional and pathological systems is used in this dialogue. The functional- pathological system is formed during a SCENAR- session and this system allows the patient’s body to make a clearer image of a disease and activate the self-regulation systems.

 The main task of a SCENAR-specialist is to look about and select the optimum tactics of operation with a patient by way of creating the cooperation system «Operator – Device – Patient» («Therapeutic Triangle»).  The diagnosis is not important in SCENAR- therapy but an ACTIVE COMPLAINT “here and now” is important (maximum subjective discomfort during a treatment session): localization, character, intensity and duration of pain or other subjective sensations.

The tactics to be applied while operating with a patient shall be determined depending on whether an acute or chronic state caused his/her complaints.

Homeostasis System Organism System parameters Adaptation mechanisms Acute disease System lapse parameters Health status SCENAR

Selection of a stimulation mode in acute state  Basic operation mode – Subjectively – Dosed Mode (SDM). Its purpose is to obtain positive dynamics of a complaint at the earliest and to accelerate organic disorders recovery.  Additional operation mode– Individually – Dosed mode (IDM). It shall be used: - if there is no sufficient effect after the SDM had been applied, - to improve SCENAR-therapy effectiveness by way of creating difference of the patient’s body stimulation.

Adaptation mechanisms Acute diseaseChronic desease Health standard Adaptation mechanisms Pseudo health standard Homeostasis

 The body can stop perceiving its complaints as a problem and treat it as a new norm variant during the adaptation process.  Basic tasks of SCENAR-therapy in chronic state: - to help the patient’s body to restore adaptive mechanisms to the normal physiology level, - to help a patient to change his/her opinion on chronic state as a norm variant, - to help the patient’s body to complete an adaptive reconstruction and cope with a problem.

Health standard Adaptation mechanisms SCENAR treatment of chronic diseases Pseudo health standard SCENAR application Homeostasis Adaptation mechanisms

Selection of a stimulation mode in chronic state aggravation  Basic operation mode – Individually-Dosed Mode (IDM). Its purpose is to help the body to make an adaptive reconstruction and obtain positive dynamics of a complaint.  Additional operation mode – Subjectively-Dosed Mode (SDM). It shall be applied: - for additional stimulation within the local zone, - to improve SCENAR-therapy effectiveness by way of combination and alteration of various stimulation of a patient’s body.

During the treatment, the SCENAR physician monitors responses of the autonomic nervous system, immune, hormone and neuropsychic systems. The selected stimulation result determines an each individual algorithm to be applied in order to select a mode and zones to be stimulated, to select stimulation parameters, time and repetition of stimulation. The individual approach significantly improves the operational effectiveness of the system «Operator – Device – Patient».

The biocybernetic “BODY – DEVICE” system The SCENAR technology incorporates a cybernetic system of monitoring of the therapeutic treatment and optimum adjustment of the signals. The system follows the rhythms of the body, operating in harmony with the body as an integral part of it and providing real-time adjustment. The body of a patient gradually leans how to function properly, depending on the situation and the genetic program.

The Specialist’s task in the system «Operator- Device – Patient» is to observe the treatment process, correct the patient’s current state and control this system efficiency. The Patient’s function in this system is to observe carefully and actively the dynamics of complains and health during the therapy, to perform recommendations given by the specialist and change its way of life properly. The function of SCENAR Device in the system is to indicate informational areas for therapeutic stimulation, to form therapeutic impulses and correct them during the stimulation process.

Conclusions: 1.The core of a disease is formed by different disorders of homeostasis. Therefore, treatment should be targeted at restoration and normalization of functions of the adaptation systems supporting the homeostasis. 2.The new medical technology appeared at the junction of holistic and official medicine; and this technology combines the philosophy of a man harmonization as one whole system, as a personality and modern technical solutions. 3.SCT, as such, is a program for restoration of adaptation systems.

4.The high therapeutic effect of SCENAR- therapy is obtained : а) thanks to electrical stimulation parameters provided by SCENAR device: Electrical impulse has non-damaging physiological parameters and can change them if there is a body’s reaction, Adaptive features of SCENAR device allows to form the biofeedback during the stimulation. As a result, the control of its stimulation correction is transferred from the device to the patient’s body.

b) thanks to methodology that consider physiological and energetic capabilities of the body. с) thanks to formation of the treatment system «Operator – Device – Patient» for effective recovery of the adaptation systems. Each element of this “therapeutic triangle” is an active member of the team working towards the same goal.

High effectiveness of SCENAR therapy is explained by the compliance with the main requirement of medicine – treatment of the patient rather than the disease:  patient-tailored approach: - individual zones of stimulation, - individually selected duration of stimulation, - treatment based on the personal complaint of the patient, - treatment principle “here and now”, - responsiveness of stimulation treatment (individual changes in the treatment pattern selected for the patient).  activation of all constituent elements of health (somatic, psycho-emotional and intellectual).
