Role of the PRC

Why is Caregiver Support missed? Hard to see if you aren’t looking for it!

What’s new… Education / Network Building

Knowledge Translation in NBRHC 4 presentations (2 in each of Sudbury & North Bay) 2-hour presentation focused on CGPL for administrators 3-hour presentation focused on SPRG for front-line workers Total number of participants: 61 Professional/Organizational affiliations: Local Health Integration Network Behaviour Supports of Ontario NE Dementia Network Coalition Community Care Access Centres Alzheimer Society Seniors’ Mental Health Program Long-term care homes First Nations Huntington’s Society Parkinson Society North Bay Regional Health Centre …others

Process Invitations sent across all of northeast to admin / management, policy planners, educators, regulated & unregulated health professionals … hospital, community / home care service providers, long term care, seniors mental health, allied organizations (Alz Society, etc.) ~70-80 representatives in total Follow-up reminders sent week before presentations

Initial Feedback Evaluations returned: 44 (out of 61) Overall organization of Toolkit: 9/10 Content of CGPL: 8/10 Content of SPRG: 9/10 Level of interest in using CGPL: 8/10 Level of interest in using SPRG: 8/10

Comments “Very timely and related to a lot of initiatives going on provincially in Ontario. Caregiver/provider input is often neglected or minimal at best; this will definitely reinforce the value of caregivers - formal and informal.” “This is a great tool! I plan on sharing the tool and resource manual information within our organization. I see an immediate use of the toolkit.” “Thank you! Those of us who work with family members/caregivers often know the importance of advocating for them. This resource is a way to quantify and advocate for this group”. “Very timely information for me as we implement Behaviour Supports Ontario Project”. “I am both a service provider and a caregiver. It was amazing to see both of my experiences reflected in the Toolkit. Thank you so much.”

Comments “I believe someone already mentioned it but the layout of the website - colour, font and organization I feel could be better. Dark background colour makes it difficult to read. It seems cluttered and I don't want that to effect someone's perception of the tool or ability to navigate it”. “Not sure if this is applicable to what I am currently working on”.

Outcomes Successes: Participation / interest (4 workshops) PRC Involvement (Sudbury / North Bay = consistent support) “Senior Friendly Communities”, “Seniors at Risk”, “DN”, “GNA” linkages NEDNC role ??? Toronto: March 15 / 16 … enhancing “understanding” Challenges: “climate of change” (baby boomers retiring: “leaders”; BSO initiative, staff turnover) Distance Time ($$$ ?) Increasing awareness / understanding of the SPRG resources Diversity of service providers/silos

Linking with Partners in Care

Next Steps – Dissemination & Uptake Local Health Integrated Network Webinar interest Dementia Knowledge Exchange Webinar interest Northeastern Ontario Stroke Collaborative Conference – Toronto 2012 North Shore Tribal Council – LTC Program Connection to Behaviour Supports of Ontario Northeast LTC Family Council Conference Gerontological Inter-oganizational/Inter-professional Collaboration Local dialogue / initiatives via PRCs, PECs, DNs, NEDNC, etc. “regional & district based” Outcomes … “work in progress”

Northeast Dementia Network Coalition (NEDNC) The goal of Dementia Networks is to:“facilitate the collaboration of available people and resources to improve the system of care of persons with dementia and their families and caregivers.” The purpose of the North East Dementia Network Coalition is to be a unified voice for the 6 Dementia Networks in Northeast Ontario, … Local Health Integration Network (LHIN).

Outcomes: Dissemination & Uptake Ensuring “Caregivers voices are heard” (NB DN Caregiver/Family – Service Provider) … future planning Key Partners to Engage: - BSO Teams, PRCs, Service Providers (across “Continuum of Care”: Interim Care Units, LTCHs, CCAC, SMHP, Retirement Homes, Home Support, Alz Society / Parkinson Society) - Best Practice Champions, Peer Leaders, Management, … “unregulated” healthcare providers, too! - Informal caregivers: family, volunteer groups, community - building on existing partnerships Services vs. Gaps “SWOT analysis” … avoid the “service silos”
