IN - HPRP Initial and Quarterly Performance Reports The Initial Performance Report covers the period from the grant execution date to September 30, Due to IHCDA by October 2 nd. Contains one time supplemental information, as well as the QPR.

IN - HPRP QPR Requirements: Unduplicated count of persons and households served via an HMIS. Persons served is divided into Prevention vs. Rapid Rehousing. Services are reported by person by category, e.g. 10 persons received moving assistance. Housing status at intake and discharge. Due to HUD 10 days after the end of a quarter. To IHCDA within 3 days.

IN - HPRP QPR: Housing Outcomes – total number of persons exiting HPRP by housing outcome. Housing outcome types – permanent, temporary, institutional and other. Total # of persons leaving – includes deceased, don’t know and missing.

IN - HPRP Training Registration: Five attendees per sub grantee. Select Case managers who can be trainers. Complete form sent electronically and return to Lynn Morrow at IHCDA Different personnel may attend the Thursday financial track – in addition to your case managers.

IN - HPRP HMIS: The HPRP license is distinct from the HUD McKinney- Vento and/or the Family Development Network HMIS. Users of other HMIS program types will have to log off of one and log on to the other. Specific HPRP HMIS training will occur during the 3 day September training but will also be available via on line webinars. HMIS HPRP agreements will be sent out early next week. Please begin to discuss needed user licenses.

IN -HPRP Clarifications: The total length of time any HPRP assistance may be rendered to a participant is 18 months. This applies to all Housing Relocation and Stabilization Services as well as to direct Financial Assistance. Each Sub grantee is required to maintain documentation of essential eligibility criteria, e.g. income verification, habitability inspections, etc. A standard file format will be defined to facilitate auditing.

IN - HPRH Clarifications: The IHOP will be available in October, Separate training will be provided. A HPRP participant maintains their homeless status while in the program and can be qualified for other long term programs while receiving HPRP assistance. Prevention in the HPRP is based on the immediacy of the risk of homelessness. This is HUD’s “but for” test. Therefore, being on a Section 8 waiting list is not an automatic qualification for HPRP.
