NRC Perspective of Recent Configuration Management Issues Tom Farnholtz Chief, Engineering Branch 1 Division of Reactor Safety, Region IV June 2014 CMBG Conference 1

Topics What We Do and Why We Do It Design Control Design Basis Licensing Basis Latent and Current Performance Issues Being Identified Vendor Oversight The Changing Demographics 2

Implement the ROP - Inspect and Assess Focus on Engineering Organizations Engineering is the Keeper of the Design Basis The Integrity of the Design Basis is the Underlying Assumption for all Licensing Actions Maintenance of our Credibility 3 What We Do and Why We Do It

Design Control 10 CFR Part 50, Appendix B, Criterion III, Design Control Applicable regulatory requirements correctly translated into specifications, drawings, procedures, and instructions Appropriate quality standards are incorporated and deviations from such standards are controlled Measures shall be established for the identification and control of design interfaces and for coordination among participating design organizations 4

Design Basis Design basis is the information that identifies the specific functions that a facility’s SSC is to perform and the specific values or range of values chosen for controlling parameters as reference bounds for design. These values may be: Restraints derived from generally accepted “state-of- the-art” practices for achieving functional goals or Requirements derived from analysis of the effects of a postulated accident for which the SSC must meet its functional goals 5

Licensing Basis The Current Licensing Basis (CLB) is the set of NRC requirements applicable to a specific plant and a licensee’s written commitments for ensuring compliance with and operation within applicable NRC requirements and the plant-specific design basis (including all modifications and additions to such commitments over the life of the license) that are docketed and in effect. Changes to the Licensing Basis - 10 CFR 50.59, “Changes, Tests, and Experiments” - License Amendment Requests 6

Latent and Current Performance Issues Being Identified Lack of an Analysis to Show Adequate Voltage to Auxiliary Feedwater Steam Generator Isolation MOV Inappropriate Ultimate Heat Sink Spray Pond Mission Time Utilized in Operability Determinations Failure to Adequately Verify that NSSS Instrumentation Loads Would be Capable of Operating at Minimum Inverter Output Voltage Failure to Maintain Design Control of the Safety Injection Pump Room Cooler 7

Latent and Current Performance Issues Being Identified - Continued 8 Failure to Properly Evaluate the Effects of Combustion Air Temperature on the Diesel Generator Capability Failure to Evaluate the Impact on the Auxiliary Feedwater Pump Motor That Resulted From Predicted Maximum Flow and Maximum Frequency Conditions Inadequate Configuration Control Resulting in Inadvertent Isolation of Fire Water to the Vast Majority of the Plant

Latent and Current Performance Issues Being Identified - Continued Unplanned Power Excursion During Plant Startup and Power Ascension Work in Progress Resulting in Rendering Plant Equipment Inoperable Equipment Material Condition and Personnel Safety Concerns 9

Vendor Oversight Significant Challenges in the Area of Vendor Oversight Can and Have Had Negative Effects on Configuration Management and Design Control Essential Service Water System Modification to Address a Longstanding Water Hammer Issue Inadequate Technical Knowledge Resulting in Vendor Errors and Equipment Damage 10

The Changing Demographics An Amateur Built the Ark, Experts Built the Titanic Payton Manning Address to the University of Virginia Graduating Class of 2014 May 17,

Key Messages Configuration Management Encompasses Many Aspects – Design Control is a Major Focus for the NRC We Continue to See Examples of Inadequate Design Control – Both Current and Latent Vendor Oversight is Critical Changing Demographics is a Challenge 12
