Funding Source In November 2004, California voters passed Proposition 63, the Mental Health Services Act (MHSA) MHSA is intended to "transform the public mental health system.“ The goal of the Act is to create a state-of-the-art, culturally competent system promoting: wellness; recovery for adults and older adults with severe mental illness; and resiliency for children with serious emotional disorders and their families. Summary of Key Details Population Served: – Adults 26 to 59 years of age – East and Central County residents – MediCal eligible as well as uninsured – Clients with serious mental illness meeting medical necessity (see page 5 of the RFP) – Services must be 24/7 Contract limit is $579,000 Expected contractor will serve adults concurrently (see page 6) Funding period: 12 months Anticipated start of contract: July 1, 2013

Purpose of the RFP (see pages 4-7) To establish an Adult FSP in East County for East and Central County residents Population Served is expected to be experiencing: – serious mental illness – homelessness, hospitalizations, visits to PES, and/or involvement in criminal justice system – chronic physical health issues and/or co-occurring substance abuse issues

Program Approach FSP programs are capable of providing a range of services beyond the scope of typical outpatient mental health services FSP programs include: – intensive outreach – transportation support – educational and vocational support – housing support – employment support – peer support and advocacy – use of MHSA flexible funds as necessary – overall coordination of services FSP participants must have the support of a service provider 24 hours a day, 7 days a week Referrals approval by County staff Program Model: Assertive Community Treatment (ACT); see pages 6-7 of RFP

RFP Review Process (see page 11) Failure to meet any one of the following criteria will not be reviewed further: - Proposal was received by due date. - All sections of Proposal as outlined in RFP are included within specified page limits (Excluding Attachments). - Attachments are included and are complete. Panel of reviewers will score each proposal Maximum of 100 points for each proposal is possible Scoring: Work Plan Narrative 40 Points Budget 20 Points Applicant Qualifications40 Points In order to be considered for an award, the proposal must have a minimum score of 75 points
