2 DRAFT Shared Nationwide Interoperability Roadmap The Vision Broad-scale learning health system Expand interoperable data, users, sophistication, scale Nationwide ability to send, receive, find, use a common clinical data set

Why does interoperability matter? Individuals and providers need access to the right information at the right time in a manner they can use to make decisions that impact their health regardless of geographic or organizational boundaries Typical Medicare beneficiary receives care from 2 primary care providers and 5 specialists each year Only 10-20% of health outcomes are attributable to health care Information needs to flow inside and outside the care delivery system to support health 3

What does the Roadmap say about NIEM? Send, receive, find and use common data set to improve health and health care quality States and other stakeholders across the ecosystem to further explore and determine the role that NIEM can serve with regards to supporting health care and human services interoperability 4

What is NIEM? NIEM—the National Information Exchange – Collaborative partnership – Offers a common vocabulary and an online repository of information exchange package documentations (IEPDs) to support information sharing – Enables enterprise-wide information – Flexible – Exchanges are reusable – Works with existing standards 5

Why is NIEM Being Considered? Only 10-20% of health outcomes are attributable to health care. Other factors include: – Genetics – Social – Environmental – Diet and Exercise – Economic Information needs to flow inside and outside the care delivery system to support health 6

Some early examples of how NIEM may be used related to Health Care? Some states are currently using NIEM to pilot State-to-State exchange between PDMP registries. The CMS Federal Hub that authenticates individuals for the Health Marketplace subsidy and Medicaid eligibility uses NIEM to establish exchange between the States, CMS, IRS, SSA, & DHS. 7

NIEM Health Domain Stewardship Background Health is a domain in the National Information Exchange Model (NIEM). ONC was the previous NIEM Health Domain Steward. The agreement between NIEM and ONC has expired and FHA has been asked to take over the role. Process NIEM Domain Stewardship requirements are well-documented and established. Decision & Next Steps If FHA is approved to take on the Health Domain Stewardship, FHA would adopt the current structure, which extends the scope of stakeholders for NIEM purposes, and obtain resources to provide stewardship and modeling support. 8
