Derek Shepherd Derek Shepherd (Played by Patrick Dempsey) He is as the new head of neurosurgery in Seattle Grace Hospital. Shortly after arriving, he meets Meredith Grey and has a drunken one-night stand with her, only to discover that she is a new surgical intern at the hospital where he has recently begun working.

Alex Karev Alex Karev (played by Justin- Chambers) -Position: intern He initially makes a poor impression on his fellow interns, and taunts Izzie Stevens when he discovers she used to be a lingerie model.

George O'Malley George O'Malley (Played by T.R. Knight) -Position: intern He is one of Meredith’s roommates who is also the resident in Seattle Grace hospital. He earns the nickname "Bambi" because he is petite and sensitive. He is always mocked by Alex.

Izzie Stevens (Played by Katherine Heigl) -Position: intern Izzie used to be a lingerie model. She is "immensely kind" and patient. She and George move in with Meredith and become best friends.

Miranda Bailey Miranda Bailey (played by Chandra Wilson) - Position: Resident Miranda Bailey is straightforward, tough; quick-witted. She referred to as “The Nazi” by most of the interns, because of the way she treats them. She has “Rules” which must be followed at all times. But underneath she is a compassionate person.
