Meeting our Goals 2 Family Practice Physicians PHCC Practice Support Recrui tment Attachment Island Health Regional & Provincial Health Partners Unattached Patients Community Support & Community Resources Building Primary Care Capacity within the South Island Division Region, supporting stronger partnerships and attaching patients to a primary care home.

Vision Primary Health Care Centres (PHCC) will be places where local physicians take leadership for interdisciplinary health care services where residents have improved access and where providers have greater professional satisfaction. 3

Challenges of Small Practices patient access accommodating urgent same day patients afterhours care No economies of scale cannot afford to have allied care professionals practice coverage time for CME, vacation, etc. Isolation & loss of camaraderie decreased job satisfaction 4

What we learned.... Studies are showing that physicians working in larger clinics are happier. Larger clinics are more likely to have access to better equipment – economies of scale. Larger clinics can accommodate CME But.... There is a lack of business model templates Deficiency in capital funding 5

Goals of the Project PHCC Feasibility Model Project 1. Site vision definition 2. Target acquisition/operating model 3.
