This could be due to age, physical or mental illness, addiction or disability.  Other family members  Neighbours or friends  Any gender or any age  Daughters, Sons  Parents (adult or children)  Wives or Husbands

Carer’s Health and Wellbeing Check Programme Bideford Medical Centre is taking part in the Carer’s Health and Wellbeing Check Programme The surgery will be offering a free and confidential health and wellbeing check to anyone who is a carer and over 18 years old.

What will the checks consist of ?

Identify more carers – increase the surgery’s register of read coded carers To find out what support carers really need Improve the carers health and wellbeing Provide additional support and resource to the surgery’s carer population Submit surgery statistics to programme evaluation to assist with creating a National Carers programme What is the objective of the programme?

Ask patients if they are a carer or care for someone Encourage patients to register as a carer Supply your administrative lead with any names of carers or people who you think may believe to be a carer What can you do to help?
