Ruminant Gelatin BSE affected regions (Bone derived) If: Country of origin of raw materials (bone only) is BSE affected, Then: Permit will include affidavit requirements or importer responsibility restrictions

Import Permit Application Process: Complete the application (VS form 16-3) Submit completed application by mail, fax, or on-line at www.aphis.usda.gov “Apply for Import Authorization” Submit required user fees Permit issued within 2-4 weeks Permit duration is One calendar year
