Personal Care Home Any dwelling, whether operated for profit or not, which undertakes through its ownership or management to provide or arrange for the provision of housing, food service, and one or more personal services for two or more adults who are not related to the owner or administrator by blood or marriage. Personal Services includes but is not limited to, individual assistance with ambulation and transfer, and essential activities of daily living such as eating, bathing, grooming, dressing and toileting.
Community Living Arrangement Means any residence, whether operated for profit or not, that undertakes through its ownership or management to provide or arrange for the provision of daily personal services, supports, care or treatment exclusively for two or more adults who are not related to the owner or administrator by blood or marriage an whose residential services are financially supported, in whole or in part, by funds designated through the Department of Behavioral Health and Disabilities Division (DBHDD).
Assisted Living Community Assisted Living Care means the specialized care and services provided by an assisted living community which includes the provision of personal services, the administration of medications by a certified medication aide and the provision of assisted self-preservation. Assisted self-preservation means the capacity of a resident to be evacuated from an assisted living community to a designated point of safety and within an established period of time as determined by the Office of Fire Safety Commissioner.
Assisted Living Community Assisted Self-Preservation is a function of all the following: The condition of the individual The assistance that is available to be provided to the individual by the staff of the assisted living community. The construction of the building in which the ALC is housed, including whether such building meets the state fire and safety requirements applicable to an existing health care occupancy.

Dark blue is PCH – represents increase 128… obviously coded some reports from facilities as complaints as opposed to self reports. Bright green – health providers, hospitals, ERs, etc. Light Blue is former staff Bright yellow represents state agencies – MHDDAD, DFCS, APS, etc.
