Whats Changing for 2013 You do not need to actively enroll this year unless you want to: Change your coverage options Enroll in Flexible Spending Accounts (FSAs) Some changes to medical, prescription, life and AD&D and disability for 2013.

Whats Changing for 2013 Medical plan changes One medical plan offered. –Fully-insured, Traditional Plan with Deductible administered through UnitedHealthcare (UHC). Consumer Advantage Plan will be discontinued December 31, –If you currently are enrolled in this plan – and you do not make an active election – you automatically will be enrolled in the Traditional Plan with Deductible for –Beginning January 1, 2013, you will no longer be able to make contributions to your Health Savings Account (HSA). However, you may continue to use the existing funds to pay for eligible health care expenses. Read more about this change and the Health Savings Account fees in your newsletter.

Whats Changing for 2013 Life/Accidental Death and Dismemberment (AD&D) changes Carrier is changing from Minnesota Life to CIGNA No plan design changes No premium increase Open enrollment for all employees / no EOI required for coverage under guaranteed issue amounts –Optional Life $500,000 –Spouse Life $ 50,000 Short Term Disability/Long Term disability changes Carrier is changing from Liberty Mutual to CIGNA No plan design changes Still company paid

Whats Changing for 2013 FSA/Health Care Spending Account change You can contribute any amount from $50 to $2,500 to the Health Care Spending Account. Due to health care reform legislation, the annual maximum contribution amount was reduced. Hyatt Legal Plan change Plan will end effective 12/31/2012

Prescription Drug Plan Review If you elect medical coverage for yourself or your eligible dependents, you will automatically receive prescription drug coverage through OptumRx. The table on the next slide provides an overview of the prescription drug coverage.

Prescription Drug Plan Review Mail order no longer mandatory for maintenance drugs Generic drugs no longer mandatory Current prescriptions for maintenance drugs will not transfer (new prescription will be required for OptumRx mail service) Some drugs can require Step Therapy or Prior Authorization

Flexible Spending Accounts (FSA) You can contribute any amount from $50 to $2,500 to the Health Care FSA. Due to health care reform legislation, the annual maximum contribution amount was reduced. You can contribute any amount from $50 to $5,000 to the Dependent Care FSA. Reminder: Must make a new election each year

Benefit Reminders Elections are final and can only be changed if you experience a qualified change in status, such as divorce, birth, etc. –You must notify YBR within 31 days of the event Dependents include your spouse, certified domestic partner and your or your spouses/domestic partners dependent children up to age 26 –Any new dependents or previous dependents not certified must be verified before coverage will become effective
