Dementia Partnership Board A whole systems, multi-agency board for driving forward Surreywide change, service improvements and innovation. Includes primary, community and secondary care, social care, voluntary sector, carers, private providers, DLIG representatives. Lead on the implementation of the Surrey Dementia and Mental Health in Older People’s Strategy. Able to monitor local investment (eg Mental Health Liaison in acutes and virtual wards) and maintain a strategic overview of work in Surrey.

Includes primary, community and secondary care, social care, voluntary sector, carers, private providers. Local action plan based on the Surrey Dementia and Mental Health in Older People’s Strategy, able to respond quickly to national and local opportunities eg the Dementia Challenge Fund. Able to monitor local investment (eg Mental Health Liaison in acutes and virtual wards) and problem solve locally viable solutions.
