CPRP Programs Community Support Teams B,C,D & ACTION Team approach used Focus on transitional services (evolved from long-term) Average of 1-2 contacts per client per week Clients transfer In from Intake and from other existing Teams Clients transfer out to more Intense level of service (ACT) OR to successful discharge Disease Management 3700 team MoHealthnet paid (not from DMH allocation) Target those with M.I. diagnosis but not in services Very high users of public healthcare—identified/referred by DMH Team approach used Rate of contact varies Some transferred to other teams

Supported Residential Programs Sullivan Street– CJ’s Place  Serious Mental Illness AND Active substance Addiction &/or HIV+ Permanent supported housing, 18 single room occupancy units, HUD 811 supported, Places at Page  Serious Mental Illness Permanent supported housing, 23 units mixed SRO, 1 and 2 bedroom, HUD shelter + Care subsidy Has respite rooms for short-term stays West Pine Group Home  Serious Mental Illness, Residential Care Facility, transitional group home HUD 811, 9 single-person bedrooms, shared kitchen and bathrooms All have: On-site Intensive Residential Treatment staff + Community Support staff Focus: Teach/support via Psychiatric rehabilitation Daily Living skills leading to greater independence Safe/secure living environment limiting visitors and overnight guests

Housing Department Manage housing subsidies funded by HUD or various grants 1 F/T staff person 6 different grants Currently 91 clients receive a subsidy All live in apartments or houses throughout St. louis City and County Creating leasing contracts Negotiating with landlords Apartment inspections to ensure the meeting or regulations / standards Completion of required documentation for H.U.D./ DMH / Granting agencies Development of Budget Assisting in submission of funding requests Monitoring of budgets – leases and utility assistance Creation and submission of reports to DMH/HUD and other grantors
