6 MRT Supportive Housing Overview
Current Capacity: 42 capital projects funded (8 open) 8 active rental subsidy programs 7 operational pilot programs; 3 in contracting stage 1 program provides operating subsidies (NYSSHP) Currently funding 10,487 units Populations: MRT Serves high-cost, high-need individuals The exact eligibility varies by population and program Populations include individuals transitioning from an institutional facility (including hospital long-stay or a nursing home) and homeless/unstably housed individuals who are elderly or who have a serious mental illness, physical disability, HIV/AIDS, substance use disorder, or other chronic condition

7 Located at East 99th Street and Second Avenue adjacent to Metropolitan Hospital
10-story new affordable housing project that includes 176 one bedroom and studio apartments. This innovative development is a result of collaboration between HHC, HPD, NYHCR, NYCHA and was the first development in the New York State to utilize Medicaid Redesign (MRT) funds.

8 1191 Boston Road, Bronx, NY Designed to help improve the lives of high cost users of health care through the use of Medicaid Redesign Team financing, Boston Road provides 154 units for formerly homeless single adults - many living with HIV/AIDS or special needs - and low-income working adults from the South Bronx community. The ground floor houses building common areas, offices for supportive services, and recreational space including an exercise room, computer lab, and landscaped

21 units for homeless adults with histories of substance abuse and/or mental illness in Rochester MRT (HHAP), HCR (HTF), City of Rochester (HOME), NYSERDA
