Role of Regional Support Team To provide recommendations and assistance in resolving barriers to the most integrated community setting consistent with an individual’s needs and informed choice. SA Section III.E.2. and IV.D.3.

provide informed choice of providers RST Referral Process: Community Case Managers (CMs): support Individual Support Teams in resolving barriers/gaps in services and supports provide informed choice of providers complete and send referrals to CRCs SA Section III.C.5.

RST Referral Process: Community DMAS: provides technical support to CMs and CRCs assists CMs and CRCs in resolving barriers to services and supports before referring to RST processes requested changes in services resulting from RST process SA Section III.E.1-2.

RST Referral Process: Community Referrals to CRCs, then to RSTs occur if: a. the Individual Support Team (IST) is unable to locate services and supports within 3 months of enrollment into Waiver. Section III.E.3.a.

RST Referral Process: Community Referrals to CRCs, then to RSTs occur if: b. the IST recommends an individual living in his/her own home, a family home or sponsored home move to a group home with 5 or more individuals. Referrals would be made if moving to a congregate location with 5 or more individuals. This criteria is considered rare for individuals with DD Waiver. Section III.E.3.b.

RST Referral Process: Community Referrals to CRCs, then to RSTs occur if: c. The IST recommends that an individual move into a nursing home (NH) or Intermediate Care Facility (ICF). Section III.E.3.c.

RST Referral Process: Community Referrals to CRCs, then to RSTs occur if: d. there is a pattern of an individual being removed from his or her home. This might be someone who has an ongoing medical or crisis need. Section III.E.3.d.

Referrals to RSTs from CRCs also occur: RST Referral Process: Community Referrals to RSTs from CRCs also occur: e. whenever [a CRC] believes that external review is needed to identify additional steps towards more integrated settings and informed choice. The CRC may consult at any time with the Regional Support Team. Upon referral to it, the Regional Support Team shall work with the Personal Support Team ("PST") and CRC to review the case, resolve identified barriers, and ensure that the placement is the most integrated setting appropriate to the individual's needs, consistent with the individual's informed choice. The Regional Support Team shall have the authority to recommend additional steps by the PST and/or CRC. Section III. E. 2 Section III.E.2.

Actions taken by IST/CM RST Referral Process Referrals provide information on: Individuals preferences & good life Supports needed and planned Actions taken by IST/CM CRC agreement with referral Notification of informed choice and opportunities for integrated settings

RST Referral Process Members of the Individual Support Team or DMAS may be invited by CRC or the RST to assist with some discussions.

RST Referral Process RST Actions: Reviews good life and confirms informed choice. Reviews options explored/considers alternatives Assures information about community integration and opportunities provided Considers individual needs and preferences and most integrated settings

RST reporting RST recommendations and resolutions documented on the referral form by the CRC. Data on reasons for referral, barriers, recommendations and resolution collected.
