Annual health checks for people with learning disability North East Learning Disability Clinical Network July 2012

NE Learning Disability Clinical Network Was formed 2½ years ago Made up of clinicians, commissioners, Learning Disability Partnership Board representatives ‘To improve the health and wellbeing of people with learning disabilities in the north east and eliminate avoidable premature deaths, injury and illness’ Has been very successful on developing regional initiatives

Some examples of success Coding & flagging systems in acute hospitals to identify patients with learning disability so reasonable adjustments can be made Care pathways for patients with learning disabilities being implemented in every acute hospital North East Regional Access to Acute Network Care of a Patient with a Learning Disability in the Acute Hospital

Annual Health Checks – what we want to achieve Standardise processes for Annual Health Checks in General Practice Easier production of Health Action Plans More effective communication of individual patient needs between primary & secondary healthcare Through standard clinical content & data collection.

Development ethos: We have made every attempt to include as many interested parties in the development process. LD DES Feb 2012 – “Practices taking part in the DES should base their health checks on the Cardiff Health Check protocol… or a similar protocol agreed with their PCT. Health checks should integrate with the patients’ personal health record or health action plan.” We have sought existing “best practice” from across NME as well as running workshops for interested NHS professionals and attending meetings with subject matter experts and specialists in related fields.
