Figure 2 A, Three frame posts were first sutured to the subcommissural regions to orient the device vertically and horizontally. B, After insertion, looping horizontal mattress sutures were placed around the body of the frame and then vertically through the valve annulus. After all sutures were placed, they were tied over small custom-designed Dacron pledgets that were narrow enough not to extend out to the leaflet tissue. Initially, small bites of Dacron were taken, but the method seemed to work better (and proceed faster) with just looping of the frame, eliminating the Dacron bites.
